How does a patient or family get a referral?

Anyone is welcome to call Memorial Hospice for more information about hospice for themselves or a loved one. To receive hospice services, a physician must make a referral.

You don't need a doctor's referral to contact us; we'll arrange for the physician's order approving Hospice care so we can begin to care for you or your loved one as soon as possible.

When should someone seek hospice?

There is a diagnosis of an illness which will result in a limited life expectancy (usually six months or less).
Curative measures are not being sought, although a patient or their family may change their mind at any time to pursue active treatment.
There is concern about managing care in the future, and the family wants to explore long-term options.
There is a need for comfort care, pain and symptom management.
