Memorial Transitions Program offers comfort-focused services to adults with life-limiting illnesses and their families. We provide information, resources and support to help clients plan for anticipated life changes and make informed decisions about their care.
Why Transitions?
It's difficult living with a life-limiting illness. Transitions supports clients and families as they face medical challenges and other life changes. We help clients to identify needs and obtain assistance as necessary.
Memorial Transitions offers customized services that address the specific needs and goals of each client. Clients can receive one-time or ongoing phone support and visits at their home or other place of residence. Services are provided by the Transitions Program Coordinator and trained volunteers.

Any adult with a progressive life-limiting illness who resides in Yakima County may be eligible for Transitions. This includes those who currently receive other in-home services (such as respite, COPES or home health). Prospective clients and their physicians consult with the Transitions Program Coordinator to make a final determination of eligibility.
